All VBL forms can be downloaded below. Please note that to view the forms you will need Adobe Acrobat Reader which is available as a free plugin for your browser from www.adobe.com.

Other Competition Forms

AMCOS/APRA Declaration

Permit Application Form * (No longer required for Victorian contests under new 2010 Victorian Rules. For interstate contests the relevant form must be obtained from the State running the contest). Note that Permits are still required for the Australian National Band Championships, which are conducted under the NBCA Rules. The relevant form for the Australian National Band Championships 2012 being held in Melbourne at Easter is available either from the Nationals website via the button at the left, or from here.

Player Nomination List to be used to advise the Registrar of players to be used at a contest. (This is available as a Microsoft Word document (84 KB) which can be filled in on a computer, and also as a PDF file (27 KB) to print out for filling in in writing.) The Registrar has also issued some Contesting Guidelines to assist with the player registration process. (PDF file - 64 KB)
